• When you first arrive, please register with the receptionist.
     
   
• If this is your first appointment, plan to arrive a little early to complete your registration and insurance information. You may also pre-register online by clicking the Pre-Registration link at the top of the page.
     
   
• Have your health insurance cards with you. These may be cards from your health insurance plan, a pharmacy benefit card, your Medicare or Medical Assistance card.  The Receptionist will confirm important personal and insurance information at this time.
     
   
• Please be courteous. We ask that you do not bring food or drinks into the waiting room. Once your are in the exam room, please turn off your cell phone.
