When you first arrive, please register with the receptionist.
If this is your first appointment, plan to arrive a little early to complete your registration and insurance information. You may also pre-register online by clicking the Pre-Registration link at the top of the page.
Have your health insurance cards with you. These may be cards from your health insurance plan, a pharmacy benefit card, your Medicare or Medical Assistance card. The Receptionist will confirm important personal and insurance information at this time.
Please be courteous. We ask that you do not bring food or drinks into the waiting room. Once your are in the exam room, please turn off your cell phone.
